Return Shipping Policy

Instructions For Patients Residing Outside of the United States

It is the responsibility of health care providers and patients to ship completed test kit boxes back to Diagnos-Techs Laboratory within the time frame indicated on the requisition form. Diagnos-Techs is pleased to offer its international clients a discounted shipping rate on their return shipping through UPS. To benefit from this service, please click here and enter your information.

If you choose to return your test kit using UPS, all shipping materials you will need have been included in your kit. The following instructions explain how to return your test kit to Diagnos-Techs through UPS.

  1. Go to UPS returns , choose the service you desire, pay, then print out the shipping label, customs invoice and your receipt.
  2. Place vials (sealed inside the ziplock bag included with your kit), requisition (test) form and payment into kit box.
  3. Tuck front flap into box. Seal with packing tape.
  4. Insert the kit box into the UPS Express Pak mailing pouch that was included with your test kit. If you did not receive an Express Pak pouch with your test kit, please contact your provider.
  5. Sign your shipping label and one copy of the commercial invoice. Place all three copies of the commercial invoice and your shipping label into the "International Shipping Documentation" ziplock pouch with the shipping label and barcode visible. Seal the pouch.
  6. Peel off the backing of the "International Shipping Documentation" and affix it to the UPS Express Pak.
  7. Please send from your most convenient UPS location, or schedule a pickup through UPS by calling UPS at 1-800-PICK-UPS (742-5877). Additional charges may apply to schedule a pickup.

If you choose not to use UPS, delivery charges still apply. International deliveries should be addressed to the physical address only, listed below. Do not address to the PO Box. Deliveries can be made Monday through Friday via a private courier of your choice. Please be sure to fill out and include a customs form.

It is recommended that kits be returned in advance of or after a public holiday. For a list of US public holidays and closures, please click here.
