We are a contracted provider with Medicare. Coverage is determined by Medicare and based upon medical necessity. Your referring provider will need to be eligible to order and refer services. An Advanced Beneficiary Notice of Non-Coverage form is required for all test orders.

Please note that Medicare no longer covers salivary testing.
