Medicare
We are a contracted provider with Medicare. Coverage is determined by Medicare and based upon medical necessity. Your referring provider will need to be eligible to order and refer services. An Advanced Beneficiary Notice of Non-Coverage form is required for all test orders.
Disclaimer: Please note that Medicare covers only stool and urine panels. Saliva panels are not covered and must be paid for in full by the patient. If a test includes both covered (stool and/or urine) and non-covered (saliva) components, the patient is responsible for the portion of the cost related to the saliva test.
Patients may also choose to pay the full amount upfront and seek reimbursement from Medicare where applicable, to prevent any delay in the release of test results.
