Shipping Instructions

Before You Ship

When planning to collect your specimen, we recommend you first plan the date you will ship the test kit back to DiagnosTechs, and then collect the final specimen on the previous day or same day. This ensures maximum specimen viability. If you are collecting for a panel that requires only one day of specimens, your final specimen should be collected either the evening before or morning of the day you ship. If you have additional questions about collecting a specimen please check here for detailed instructions, or check with your healthcare provider.

  1. Write your name, address, date of birth, and collection date(s) on order form.
  2. Write your name and date of birth on all vials.
  3. Be sure required test orders are marked on the form. If not, please contact your provider for test orders.
  4. Include payment check or credit card information and copy of Medicare Part B card and ABN form if applicable.

Instructions For Patients Residing in the United States

Free Shipping for a Single Kit in the United States!

DiagnosTechs test kits contain a pre-addressed return label and include free shipping which guarantees delivery within three business days for kits shipped within the U.S.

Just place your specimens in the box, seal it shut and drop off at your most convenient shipping location corresponding to the shipping carrier label in your test kit box.

The return label included in your test kit guarantees a maximum of three days from the date you ship until it is received by DiagnosTechs. If you have lost your prepaid label please contact us to request another one. In the case of label replacement, you will not be charged for this service.

It is recommended that kits be returned in advance of or after a public holiday.

Note:  DiagnosTechs domestic test kits may contain either a FedEx or UPS return shipping label. Please drop off your test kit with the carrier indicated on your return shipping label to ensure proper acceptance.

  1. Place vials (sealed inside the bag included with your kit), order form and payment (if indicated) into kit box.
  2. Tuck front flap into box. Seal with the enclosed shipping label. Place label within dashed lines and adhere over front edge of box.
    IMPORTANT: Ensure that the portion of the label containing the bar code is not bent around any corners on the box.
  3. Please send from your most convenient shipping location corresponding to the shipping label in your kit, or schedule a pickup through the shipping carrier directly. Additional charges may apply to schedule a pickup. Note: the shipping label in your test kit may differ from the image. Please send your test kit with the carrier indicated on the return shipping label to ensure acceptance. 

 


Instructions For Patients Residing Outside of the United States

It is the responsibility of health care providers and patients to ship completed test kit boxes back to DiagnosTechs Laboratory within the time frame indicated on the order form.

Please ship with the carrier of your choice to guarantee next day or two-day delivery.

  1. Choose the carrier and service you desire, pay, then print out the shipping label, customs invoice and your receipt.
  2. Place vials (sealed inside the bag included with your kit), order form and payment (if indicated) into kit box.
  3. Tuck front flap into box. Seal with packing tape.
  4. Insert the kit box into the shipping pouch.
  5. Sign your shipping label and all copies of the invoice. Place the signed copies of the invoice and your shipping label into the shipping pouch with the shipping label and barcode visible. Seal the pouch and affix it to the package.
  6. Please send from your most convenient shipping location.

International deliveries should be addressed to DiagnosTechs as listed below. Deliveries can be made Monday through Friday. Please ship with the carrier of your choice, to guarantee next day or two-day delivery.

If you are shipping from outside of the United States, please be sure to fill out and include a customs form.
